  Item: Safety Vacuum Release System documentation must be kept in facility, if applicable. Each SVRS shall be tested monthly and the test dates shall be recorded.
  Comment: Because of your drain size and only having one, it is mandatory to have a functioning SVRS system in place and to document testing it every month. Yours does not work. Fix or replace and document the appropriate testing.

Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
