  Item: No pool or spa shall be constructed or reconstructed without the owner or a designated representative of the owner first receiving a permit from the department. Construction shall be completed within 24 months from the date the construction permit is issued unless a written extension is granted by the department.
  Comment: Spa was not decommissioned properly, and without a permit from the state of Iowa. A permit needs pulled and all of the spa supplies ()filter, heater, piping) need removed properly.

  Comment: . 1. Pool shall manually tested 30min prior to opening. I suggest to either train someone else to do this or change the hours of the pool to start after maintenance arrives for the day. 2. The readings from the controller shall be documented at 4pm and 8pm every evening by assigned staff on duty

  Item: "No Lifeguard" signage/incorrectly posted. Where lifeguards are not provided whenever the pool is open, a sign shall be posted at each entry to a swimming pool or a wading pool.

  Item: A first-aid kit shall be equipped with: band-aids, sterile 4x4 bandage compress, self-adhering gauze bandage, disposable gloves, and a chemical cold press.

  Item: Swimming pools where lifeguards are not provided shall have a designated emergency phone and shall be available to users of swimming pools when the swimming pool is open. When the telephone is not within the confines of the swimming pool, the location of the emergency telephone shall be posted in a conspicuous place within the swimming pool enclosure. Instructions for emergency use of the telephone shall be posted near the telephone.
  Comment: Post sign that indicates the address of the facility by emergency phone.

  Item: All drains shall be VGB compliant and within their life expectancy date
  Comment: Existing drain covers appear to be without cracks or missing screws. But, my records indicate that they expired 11/4/2022. If that is the case, replace asap!

  Item: The facility management shall develop a written emergency plan. The plan shall include, but may not be limited to, actions to be taken in cases of drowning, serious illness or injury, chemical-handling accidents, weather emergencies, and other serious incidents. The emergency plan shall be reviewed with the facility staff at least once a year, and the dates of review or training shall be recorded in the pool records
  Comment: Create and have all staff sign off on this now and annually thereafter.

  Item: Material safety data sheets (MSDS) for the chemicals used at the pool or spa shall be at the facility in a location known and readily accessible to the facility staff. The SDS sheets shall be reviewed with the facility staff at least once a year, and the dates of review or training shall be recorded in the pool records

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
