2) Water Quality Pool: 15.4(3)b(1) 12
Spa: 15.51(3)b(1) 12
Item: The pool or spa shall be clean
Comment: There is prominent discoloration in certain areas of the pool floor and sidewalls. This has been an ongoing issue at this site. Ensure that these areas are power washed and/or cleaned the next time this pool is drained.
3) Safety Spa: 15.51(4)c(1) 13
Item: A thermometer shall be available to measure temperatures in the range of 80 degrees to 120 degrees. Spa shall be closed if the temperature is over 104 degrees F.
Comment: Previous records show the spa's temperature at 104 degrees which is measured by reading the temperature on the heater itself. Because this reading is unable to measure beyond the whole 104 decimal point, we do not know if it is 104.0 (acceptable) or 104.9 (unacceptable). Make sure that the temp of the spa does not exceed a reading of 103 on your heater at any given time, AND have a backup thermometer available.
4) Safety Pool: 15.4(4)f(6) 17 and 18
Item: At swimming pools where lifeguards are employed, a telephone shall be available to the swimming pool staff for emergency purposes.

Item: The facility management shall develop a written emergency plan. The plan shall include, but may not be limited to, actions to be taken in cases of drowning, serious illness or injury, chemical-handling accidents, weather emergencies, and other serious incidents. The emergency plan shall be reviewed with the facility staff at least once a year, and the dates of review or training shall be recorded in the pool records
Comment: Have all employees sign off on reviewing and understanding this annually.

Item: Fully submerged lighting in a pool or spa shall be in working order.
Comment: The underwater lights need attention. Some don't work and none of them can be kept on at this time because one of the fixtures is not mounted properly. Please ensure that all underwater lights in the pool and spa are in working order, or permanently removed. Be sure to contact IDPH prior to permanently removing any light fixtures for approval.
9) Safety Pool: 15.4(4)m(1) 17
