Notes: I modified this report on 8/27/21 after consulting with John Kelly from IDPH in regards to the waterslides. Please note violation #4.

1) Management/Personnel Pool: 15.4(6)b 23
  Item: "Pool Rules" signage incorrectly posted. Rules sign shall include No diving in shallow end, No horseplay, and No running on deck. Post conspicuously at a minimum of 2 locations within the swimming pool enclosure.
  Comment: Please add another Pool Rules sign.
   
2) Water Quality Pool: 15.4(2)a(1) 9
  Item: Pool water shall have a free chlorine residual of at least 1.0 ppm and no greater than 8.0 ppm, or a total bromine residual of at least 2.0 ppm and no greater than 18.0 ppm when pool is in use
  Comment: The logbook showed several chemical readings where the chlorine was greater than 8.0 without notice of pool closure. If the chlorine level is greater than 8.0, the pool shall be closed and the logbook shall be documented as so until the chlorine level returns to an acceptable level.
   
3) Safety Pool: 15.4(4)b(2) 13
  Item: Pool ladders, ramps, handrails, and grab rails shall be securely anchored.
  Comment: The deep-end ladder is loose. Please tighten.
   
4) Safety Pool: 15.4(4)d(5) 16
  Item: Each water slide shall have a minimum of two attendants, one stationed at the top of the slide and one at the bottom of the slide.
  Comment: Since your slides discharge from 2 different levels, they shall have (2) top attendants, 1 at the starting platform for the white slide and 1 at the starting platform for the blue slide, plus the attendant at the bottom of the slide
   
5) Safety Pool: 15.4(4)f(5) 17
  Item: A standard spine board with straps and a head immobilizer shall be provided at each swimming pool where lifeguards are required by rule.
  Comment: Clasps on spineboard straps are broken. Please replace.
   
6) Showers/Toilets/Dressing Rooms Pool: 15.4(5)e 23
  Item: Soap shall be available at each lavatory and at each indoor shower fixture.
  Comment: Please add soap to shower stall in the bathroom(s).

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
