  Comment: Sign up for next available class. Email me a copy of your certificate once obtained and keep a copy in your filter room. Spa shall remain closed until you have a CPO on staff at least 10hrs per week.

  Comment: Have all staff that works with the pool and all nursing staff that work in the building review and sign off on this now and annually thereafter.

  Item: Not tested/recorded as often as necessary in Operation Records
  Comment: Spa is to be tested prior to opening in the morning and then every 2 hours thereafter while spa is open. Consider limiting spa hours due to lack of use and inability to get someone there to do testing every 2 hours in the evenings, at night and possibly on the weekends. All maintenance should be trained on how to test and document the results in Frank's absence.
   
4) Safety Pool: 15.4(4)h 18 and 19
  Item: All main drains shall be VGB compliant and within expiration date
  Comment: Replace main drain cover with VGB approved cover. Take a picture of drain cover prior to installing it, write the date of installation on the picture, email it to me and keep in your pool binder in the filter room. Spa is to remain closed until this has been completed.
   
5) Safety Pool: 15.4(4)b(8) 14
  Item: When stairs are provided for entry into a swimming pool, a stripe at least 1 inch wide of a color contrasting with the step surface and with the swimming pool floor shall be marked at the top front edge of each tread. The stripe shall be slip-resistant.
  Comment: Add this to steps when pool is drained to replace main drain cover.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
