  Item: A written emergency plan shall be provided and reviewed annually
  Comment: Emergency action plan present at facility but not reviewed with staff for 2019 nor 2020. Please have staff review the emergency action plan and sign and date having done so for 2020 and every year after.
   
2) Management/Personnel Pool: 15.4(2)e 10 and 11
Spa: 15.51(2)e 11 and 12
  Item: Not tested/recorded as often as necessary in Operation Records
  Comment: Spa not being tested with necessary frequency. For example, on 12/27/19 the spa was not tested at the required 1/2 hour prior to opening nor at 7am or 9am. The log also indicated that the spa was open at that time. If the spa is open then testing shall be completed per requirements. Site CPO can advise as to the required testing frequencies.
   
3) Safety Pool: 15.4(4)h 18 and 19
  Item: All main drains shall be VGB compliant and within expiration date
  Comment: 2 main drain covers in pool were last replaced in April 2019 and have a 10 year lifespan. 2 main drain covers in spa are of unknown installation date. All four of these covers shall be replaced. Pool was closed at time of inspection and is not to re-open until new main drain covers have been properly installed. Be sure to indicate specific installation date in your records and ensure that the covers are changed out promptly when required.

  Item: Copies of MSDS sheets shall be available for employee review. Each member of staff shall review MSDS annually.
  Comment: No staff review of MSD sheets available during inspection. Sheets shall be reviewed annually and signed and dated by staff then kept in your records for review.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
