Smoke Free Air Act
1. Facility in compliance with smoking ban?
( If no, complete complaint form )

Notes: Scott said the backwash drain line was repaired ~2 years ago to connect waste water to City Sewer with a bigger drain line allowing water to adequately drain. There is a connection to drain backwash into near by creek, but facility does not drain into creek often and if does, they ensure all chlorine is out of water supply before.
If continue to drain backwash into creek, MUST OBTAIN A PERMIT "Storm Water Discharge" FROM DNR.
Closing wading pool after this year (ADA Requirements not meet).
