  Item: A written emergency plan shall be provided and reviewed annually
  Comment: Current emergency plan provides direction of Call 911 in case of drowning, serious illness or injury, chemical handling accidents, weather emergencies and other serious incidents. Additional information is suggested and required in the case of weather emergencies such as tornadoes or other severe weather.
   
2) Management/Personnel Spa: 15.51(5)b 16 and 17
  Item: "Spa Rules" signage incorrectly posted. Rule sign shall include Persons with medical condition, including pregnancy, should not use the spa without first consulting with a physician, Anyone having a contagious disease shall not use the spa, Persons shall not use the spa immediately following exercise or while under the influence of alcohol, narcotics, or other drugs, Persons shall not use the spa alone or without supervision, Children shall not be accompanied by an adult, Persons shall not use the spa longer than ten minutes, Persons shall not use the spa longer than 10 minutes, No one shall dive or jump into spa.
  Comment: Spa rules sign only includes medical condition and alcohol stipulation. Spa rules sign must include rules for persons with medical condition, contagious disease, immediately following exercise or under the influence of alcohol, spa usage without supervision, 10 minute time limit, and diving or jumping into spa.

  Comment: Self-adjusting weirs in pool and spa are missing. Repair or replace as needed.

  Item: Swimming pools where lifeguards are not provided shall have a designated emergency phone and shall be available to users of swimming pools when the swimming pool is open. When the telephone is not within the confines of the swimming pool, the location of the emergency telephone shall be posted in a conspicuous place within the swimming pool enclosure. Instructions for emergency use of the telephone shall be posted near the telephone.
  Comment: Emergency phone in pool area is inoperable. Repair or replace as needed.
   
6) Safety Pool: 15.4(4)h 18 and 19
  Item: All main drains shall be VGB compliant and within expiration date
  Comment: Main drains of spa and pool are expired. Replace and keep documentation of installation date and lifespan/expiration date.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
