  Item: Monthly ground fault circuit interrupter (GFCI) shall be included in Operational Records
  Comment: Records indicate monthly testing of ground fault circuit interrupter are not taking place. GFI's shall be tested monthly and recorded in operational records.

  Comment: 1)Pool and Spa open hours begin at 5:30am but testing records don't begin until 6:00am. Water chemistry must be tested 30 minutes prior to pool and spa opening. 2)Monthly calcium hardness tests not being recorded. Calcium hardness must be tested monthly.

  Item: Skimmer shall have an easily removable basket/screen upstream from any valve and have a Self-adjusting weir in place
  Comment: Spa was missing self-adjusting weir and pool was missing weir closest to spa. All skimmers shall have a self-adjusting weir in place. Replace weirs for both pool and spa.

  Item: A first-aid kit shall be equipped with: band-aids, sterile 4x4 bandage compress, self-adhering gauze bandage, disposable gloves, and a chemical cold press.
  Comment: First aid kit was missing sterile 4x4 bandage compress. Provide 4x4 bandage compress for first aid kit.

  Item: Pool ladders, ramps, handrails, and grab rails shall be securely anchored.
  Comment: Ladder entry to pool was loose. All pool ladders, ramps, handrails, and grab rails shall be securely anchored. Tighten as needed to prevent movement of ladder rails.

  Item: Spa ladders, handrails, and grab rails shall be securely anchored.
  Comment: Spa handrail was loose. All spa ladders, handrails, and grab rails shall be securely anchored. Tighten as needed to prevent movement of spa handrail.
   
7) Safety Spa: 15.51(4)l 16
  Item: The agitation system control (timer) shall be installed out of the reach of persons in the spa. The "on" cycle for the agitation system shall be no more than ten minutes
  Comment: Spa timer exceeded 10 minute time limit. Operator said new timer has been ordered and old timer is going to be replaced. On cycle of timers shall not exceed 10 minutes.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
