  Item: Main drain and Safety Vacuum Release System documentation (VGB) with dates of installation must be kept in facility. Each SVRS shall be tested monthly and the test dates recorded
  Comment: No documentation present for main drain installation or expiration. Obtain documentation or replace main drain covers and keep documentation on file.
   
2) Management/Personnel Spa: 15.51(5)b 16 and 17
  Item: "Spa Rules" signage incorrectly posted. Rule sign shall include Persons with medical condition, including pregnancy, should not use the spa without first consulting with a physician, Anyone having a contagious disease shall not use the spa, Persons shall not use the spa immediately following exercise or while under the influence of alcohol, narcotics, or other drugs, Persons shall not use the spa alone or without supervision, Children shall not be accompanied by an adult, Persons shall not use the spa longer than ten minutes, Persons shall not use the spa longer than 10 minutes, No one shall dive or jump into spa.
  Comment: Spa rules did not include required rule statements. Obtain new sign with required rule statements as listed Spa code book pages 16-17.

  Item: Swimming pools where lifeguards are not provided shall have a designated emergency phone and shall be available to users of swimming pools when the swimming pool is open. When the telephone is not within the confines of the swimming pool, the location of the emergency telephone shall be posted in a conspicuous place within the swimming pool enclosure. Instructions for emergency use of the telephone shall be posted near the telephone.
  Comment: Emergency phone was inoperable. Repair or replace phone to allow for operational use.
   
4) Safety Spa: 15.51(4)l 16
  Item: The agitation system control (timer) shall be installed out of the reach of persons in the spa. The "on" cycle for the agitation system shall be no more than ten minutes
  Comment: Spa agitation system control timer exceeded ten minute time limit. Repair/replace timer to not allow more than 10 consecutive minutes on timer control.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
