Notes: Pool is now using sphagnum moss filters alongside sand filters. First attempt to use emergency phone was unsuccessful. Second attempt proved successful, recommend replacing or conducting biweekly tests.

  Comment: Lifeguard and CPR certificates are expired. Provide new certifications and keep on file.
   
2) Safety Pool: 15.4(4)f(4) 17
  Item: A first-aid kit shall be equipped with: band-aids, sterile 4x4 bandage compress, self-adhering gauze bandage, disposable gloves, and a chemical cold press.
  Comment: First aid kit missing chemical cold compress and 4"x4" gauze. Provide chemical cold compress and 4x4 gauze.
   
3) Showers/Toilets/Dressing Rooms Pool: 15.4(5) 23
  Item: Swimming pool users shall have access to showers, dressing rooms, and sanitary facilities that are clean and free of debris.
  Comment: Bathroom stalls in men's locker room has debris on floor and are not clean. Clean floors and toilets in men's locker room.

  Item: Soap shall be available at each lavatory and at each indoor shower fixture.
  Comment: Provide working soap fixtures in Men's locker room shower stalls. Soap dispenser on wall was inoperable. Private stalls in Women's locker room do not have soap dispensers. Provide soap in stalls.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
