  Comment: Needed to add age required for adult supervision. Added during inspection. CORRECTED

  Comment: Must post "No Lifeguard on Duty" at every entrance to pool. Found sign in storage room. Post at entrance.

  Comment: Skimmer was missing basket. No replacement was available at facility. Provide basket and have extra on site.

  Comment: Free chlorine was measured at 12.4. Pool closed during inspection.

  Item: A first-aid kit shall be equipped with: band-aids, sterile 4x4 bandage compress, self-adhering gauze bandage, disposable gloves, and a chemical cold press.
  Comment: Must provide self adhering gauze for first aid kit.

  Item: Swimming pools where lifeguards are not provided shall have a designated emergency phone and shall be available to users of swimming pools when the swimming pool is open. When the telephone is not within the confines of the swimming pool, the location of the emergency telephone shall be posted in a conspicuous place within the swimming pool enclosure. Instructions for emergency use of the telephone shall be posted near the telephone.
  Comment: Facility has emergency phone however operator is unable to hear caller. New phone ordered during inspection. Invoice attached.

  Comment: Ladder was not securely anchored. Railing lower end was removed from anchor. CORRECTED

  Comment: Unable to locate MSDS sheets at beginning of inspection. Must be readily available. Located during inspection. CORRECTED

  Item: Each electrical outlet in the deck, shower room, and pool water treatment equipment areas of a swimming pool or spa shall be equipped with a properly installed ground fault circuit interrupter (GFCI) at the outlet or at the breaker serving the outlet, and shall be tested at least once a month. Testing dates and results shall be recorded in the pool records.
  Comment: GFI outlet on pool deck is not GFI. Electrician contacted during inspection. Repair as needed.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
