Notes: Great job keeping the records maintained and up to date. Keep working on record keeping. According to inspection dated 7/24/12 facility had new drain covers installed 11/11/11 with a 5 year life expectancy. In pool room a picture of drain cover was dated May 2009. Must verify when cover was installed and how long life expectancy is. Refer to violation #A2 15.4(4)h(1) pg.18 for requirements.

  Comment: Did not have the annual review of MSDS sheets in log book. All employees that handle any chemicals for the pool must annually review the MSDS sheets for those chemicals. Each employee must sign and date for verification each year as well.

  Comment: Annual review of EAP was not found in log book. All employees that handle pool procedures must annually review all emergency action plans. Each employee must sign and date for verification each year as well.

  Comment: VGB documentation was not found in the facility. Must keep all documentation for VGB drain (date of install, life expectancy, and picture of drain with life expectancy) in log book for verification.

  Comment: Could not find all monthly bacterial tests in log book. Only found June test in log book. Each month the pool is open, the facility is required to take a water test to a certified lab for a coliform test.

  Item: A swimming pool facility shall have a first-aid kit. Where lifeguards are not provided, the first-aid kit shall be prominently mounted in the swimming pool enclosure, or a sign stating its location shall be posted near the swimming pool.
  Comment: Did not find a sign informing tenants where to find the first aid kit. If kit is not prominently mounted in the pool enclosure then it must have a sign stating the location.
   
6) Safety Pool: 15.4(4)b(2) 13
  Item: Pool ladders, ramps, handrails, and grab rails shall be securely anchored.
  Comment: Pool hand rail has become loose. Must securely anchor the hand rail to prevent injury or accident.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
