Notes: Pool was closed (8/12) due to low ORP levels. 447mV for the wading pool, and 641mV for the main pool. Free chlorine was also tested >12ppm (main) with Taylor kit. Physical recheck (8/13) determined that Free chlorine was 6.29 with an ORP reading of 745mV (main), and the wading pool ORP was (822mV). Wading pool Free chlorine was tested @ 16ppm (by Pool Tech) on 8/13. Wading pool was backwashed and reached 7ppm. BOTH AREAS ARE ALLOWED TO RE-OPEN. Facility will be remodeling later this year.

  Comment: Facility was not submitting a monthly biological test to a certified lab. Each month that the facility is open, a water sample must be submitted to a certified lab to test for total coliform. Record of that water sample must be kept in the log book.

  Comment: Did not have emergency action plans that specifically addresses the following: drowning, weather emergency, serious injury or illness, and chemical spill. Must have a plan that specifically addresses each of these events, and keep these plans in the log book. Additionally each employee that works with the pool must also review and sign each year.

  Item: Skimmer shall have an easily removable basket/screen upstream from any valve and have a Self-adjusting weir in place
  Comment: Wading pool is missing a weir in the skimmer. Skimmers must have a self-adjusting weir in place to sanitize the surface water. See notes section.

  Item: Pool water shall have a free chlorine residual of at least 1.0 ppm and no greater than 8.0 ppm, or a total bromine residual of at least 2.0 ppm and no greater than 18.0 ppm when pool is in use
  Comment: Found chemical tests in log book that were outside of the allowable operating ranges, however no record of closing was documented. Must make sure staff are knowledgeable about min/max operating ranges, and act accordingly. Given log sheet with allowable ranges for reference.

  Item: Pool and Spa decks shall be maintained slip-resistant, and free of litter, obstructions and tripping hazards.
  Comment: Pool deck has become uneven in some spots by the diving board (east side facing the clubhouse). Decks must be free of tripping hazards to prevent injury or accident. See notes section.

  Item: A fence, wall, or other means of enclosure shall have no openings that would allow the passage of a 4-inch sphere, and shall not be easily climbable by toddlers.
  Comment: North fence by the clubhouse and the wood fence (NE corner) by the clubhouse both have areas that would allow a 4" sphere to pass through (>6"). Must lower fence to prevent 4" sphere to pass through.

  Item: All main drains shall be VGB compliant and within expiration date
  Comment: Wading pool drain cover has expired. Installed in 2009 and has a three year life expectancy. Drain covers must be within life expectancy to prevent entrapment and allow proper function. See notes section.

  Item: Pool ladders, ramps, handrails, and grab rails shall be securely anchored.
  Comment: Both North side handrails (closest to clubhouse) are loose. Handrails must be securely anchored to prevent accident or injury. See notes section.

  Item: Each electrical outlet in the deck, shower room, and pool water treatment equipment areas of a swimming pool or spa shall be equipped with a properly installed ground fault circuit interrupter (GFCI) at the outlet or at the breaker serving the outlet, and shall be tested at least once a month. Testing dates and results shall be recorded in the pool records.
  Comment: GFI outlet is wired wrong in the area by the filter (pool room). Outlet must be protected by a functioning GFI breaker to prevent electrocution.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
