Notes: Several items are repeat violations. Items must be addressed and corrected.

Spa circulation system has considerable air leak. Leak could be potential cause of frequent low levels of free chlorine. Repair leak.

Spa was closed at time of inspection. Spa may re-open once free chlorine levels are increased and maintained at 2.0-8.0ppm.

  Comment: Not able to verify annual review of written emergency plan. Written emergency plan was included in records.

  Item: Spa water shall have a free chlorine residual of at least 2.0 ppm and no greater than 8.0 ppm, or a total bromine residual of at least 4.0 ppm and no greater than 18.0 ppm when the spa is open for use
  Comment: Spa free chlorine was measured at 0.6ppm. Operational records indicate that spa is frequently below 2.0ppm. Spa must be maintained at 2.0-8.0 ppm and closed if below 1.0ppm.
   
3) Safety Pool: 15.4(4)f(4) 17
  Item: A first-aid kit shall be equipped with: band-aids, sterile 4x4 bandage compress, self-adhering gauze bandage, disposable gloves, and a chemical cold press.
  Comment: First aid kit was missing 4x4 bandage, self-adhering gauze, and chemical cold compress. Provide.

  Item: The lower end of each ladder rail shall be securely covered with a smooth nonmetallic cap.
  Comment: Missing escutcheon plates at both pool and spa entrance. Also missing caps on ladder in deep end of pool. Provide.
   
5) Safety Spa: 15.51(4)l 16
  Item: The agitation system control (timer) shall be installed out of the reach of persons in the spa. The "on" cycle for the agitation system shall be no more than ten minutes
  Comment: Spa control timer exceeds 10 minute time limit. Must prohibit ability of timer to exceed 10 minutes.

These items must be corrected as soon as possible in order to comply with the Iowa Department of Public Health Swimming Pool and Spa Rules. A letter regarding the actions which will be taken in order to correct all deficiencies must be submitted within 30 days to this office. If for any reason you take issue with any of the items cited regarding swimming pool rules, a variance can be requested by following the instructions in the Iowa Department of Public Health Swimming Pools manual, amended July 8th, 2009, page 57, section 641-15.7 (135I). Variance requests regarding spa rules must be made in compliance with the instructions in the Iowa Department of Public Health Spa manual, revised July 8th, 2009, page 32, section 641-15.7 (135I).
